Certain scenes, routines still stand out for me even know because they had me in physical pain the first time I saw them. Here's a couple.

Do you watch Rescue Me, Section? I'm no fan of Leary as a stand-up but he's consistently very funny on the show.

 

For me The Simpsons S4-12 are the absolute pinnacle of comedy and Lionel Hutz's reaction and subsequent explanation of his problem with Judge Schneider is the funniest thing I've ever heard. It is no exaggeration to say I laughed uninterrupted for 2-5 minutes.

I think that for real face-hurting, tear-producing, out-loud laughter the worst I've ever been was watching Eddie Izzard when he was at his pinnacle. It might have been the DVD he did in the US, or possibly the one before that, I'm not sure but he had whole ten minute chunks of pure genius where breath was hard to come by.
