If we are including actresses, then Halle Berry. Really good in Monsters Ball. Always incredibly gorgeous but in everything else I've seen her in, she gives the impression that she can't act. Some of her scenes in the X-Men trilogy are painful to watch, through a combination of bad dialogue and awful delivery.

The main fella out of American Werewolf in London. 
Quite a few who had one or two good films then fucked themselves up on drugs.
Tom Sizemore. Nick Nolte. River Phoenix. I’m sure there’s plenty more.

Christian Slater. Superb in True Romance. Cant really think of him being decent in anything else. Possibly Robin Hood but he's barely in it anyway.
