Is the game brand new, original or a copy to play with firmware? Are there any scratches on it?

If the game is new then take it back and exchange it. If this doesn't help perhaps your lazer is on the way out.

If the game is a copy to play with ixtreme firmware then perhaps cheap discs have been used. Try using Verbatims and Imgburn. If that fails then you need a new burner to burn your copies.

Think that just about covers every alternative.
